Sinus floor elevation
A procedure that lifts the floor of the maxillary sinus and its lining membrane to create a space that is filled with graft material, so that implants can be placed in the posterior maxilla where bone height is insufficient.
Two approaches are in use. In the lateral window technique, a window is cut in the lateral wall of the maxillary sinus, the Schneiderian membrane is lifted from the floor, and graft material is placed into the space created. In the transcrestal (transalveolar, osteotome) technique, the floor is fractured and lifted through the implant osteotomy, with a smaller amount of graft, and the implant is usually placed at the same time. The 2008 systematic reviews by Pjetursson and Tan, and a 2018 randomised trial on morbidity, compare the two.
Graft material in the sinus is not covered by a membrane in the way a ridge graft is; it is contained by the lifted sinus membrane above and the bony walls around it. Volume stability of the graft over the months of healing is one of the outcomes studied for materials used here.
