Sticky bone

A clinician's term for particulate graft mixed with the patient's own platelet concentrate (A-PRF, CGF or similar) so that the fibrin holds the particles together as a mouldable mass. Prepared at the chair from a blood draw.

Sticky bone is not a product but a chairside preparation. Blood is drawn from the patient and centrifuged to obtain a liquid platelet concentrate; the concentrate is mixed with particulate graft and allowed to clot, and the fibrin network binds the particles into a mass that can be shaped and placed as one piece. The idea answers the stability principle in Wang and Boyapati’s PASS framework directly: a graft that holds together is easier to keep where it was placed.

The technique requires venepuncture, a centrifuge and a protocol for the spin; the result varies with the patient’s blood and the operator. The 2026 review in Annals of Medicine and Surgery summarises the evidence and asks in its title whether the technique is a paradigm shift in graft stability or one still under scrutiny.
